Job Description
Join Amedisys as an LPN (Licensed Practical Nurse) for a rewarding opportunity to provide dedicated, one-on-one patient care in a home setting. This role offers the unique chance to build strong relationships with your patients, providing personalized medical support and comfort. You will be instrumental in managing patient health, administering medications, performing treatments, and educating patients and their families on health management, all while working autonomously with the full support of our clinical team. Key Responsibilities
- Provide skilled nursing care to patients in their homes, following a physician's prescribed plan of care.
- Administer medications, perform wound care, and conduct treatments as ordered.
- Monitor and assess patient health status, reporting changes to the Registered Nurse or physician.
- Educate patients and their families on disease processes, medication management, and self-care techniques.
- Maintain accurate and timely clinical records and documentation.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
Required Skills
- Current LPN license in the state of South Carolina.
- Minimum of 1 year of clinical experience, preferably in home health or a related field.
- Strong assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage a patient caseload effectively.
- Valid driver's license and reliable transportation.
Preferred Qualifications
- Experience with electronic medical records (EMR) systems.
- BLS/CPR certification.
- Prior experience in hospice or palliative care.
