Job Description
We are seeking a dedicated and compassionate Gerontology Nurse Practitioner – Senior Care Specialist to join our multidisciplinary team. In this pivotal role, you will provide comprehensive primary and specialty care to our senior patient population, focusing on health promotion, disease prevention, and the management of acute and chronic conditions prevalent in older adults. This position requires an advanced practice registered nurse who is passionate about improving the quality of life for seniors through evidence-based practice and patient-centered care. You will work collaboratively with physicians, nurses, social workers, and other healthcare professionals to develop individualized care plans, ensuring holistic support for our patients and their families.
Key Responsibilities
- Conduct comprehensive health assessments, including physical exams, medical history reviews, and functional status evaluations for geriatric patients.
- Diagnose and treat acute and chronic illnesses common in the elderly, prescribing medications and other therapeutic interventions as needed.
- Develop, implement, and evaluate individualized care plans, integrating principles of gerontology and palliative care.
- Provide patient and family education regarding disease management, medication adherence, nutrition, and lifestyle modifications.
- Order and interpret diagnostic tests, collaborating with specialists as necessary to ensure comprehensive patient care.
- Monitor patient progress, adjust treatment plans, and provide ongoing follow-up care.
- Participate in interdisciplinary team meetings to coordinate patient care and ensure seamless transitions across care settings.
- Maintain accurate and detailed patient records in compliance with all regulatory and organizational standards.
- Act as a resource and mentor for other nursing staff on geriatric best practices.
Required Skills
- Current, unencumbered Advanced Practice Registered Nurse (APRN) licensure in the State of Florida.
- Board Certification as a Gerontological Nurse Practitioner (GNP) or Adult-Gerontology Nurse Practitioner (AGNP).
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Demonstrated expertise in geriatric assessment, diagnosis, and treatment.
- Strong clinical judgment and critical thinking skills.
- Excellent communication and interpersonal skills, with a patient-centered approach.
- Ability to work effectively both independently and as part of a collaborative team.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- Minimum of 2 years of experience as an NP, specifically working with a senior population.
- Experience with palliative care or hospice care.
- Fluency in additional languages relevant to the patient community.
