Job Description

As a Hospice Intake Coordinator at Sue Ryder, you will be the compassionate first point of contact for patients, families, and healthcare professionals seeking our services. This pivotal administrative role requires exceptional organizational skills, empathy, and the ability to manage sensitive information with discretion. You will guide potential service users through the initial stages of their journey with us, ensuring a smooth and supportive intake process. Your work will directly contribute to our mission of providing expert and compassionate palliative care.

Key Responsibilities

  • Serve as the primary point of contact for all incoming inquiries regarding hospice services via phone, email, and in-person.
  • Conduct initial screenings to gather essential patient information and assess suitability for our programs.
  • Accurately document all intake information into our electronic health records system, ensuring data integrity and confidentiality.
  • Liaise with healthcare professionals, social workers, and internal clinical teams to coordinate admissions and service transitions.
  • Provide empathetic and clear communication to patients and families, explaining services, eligibility criteria, and next steps.
  • Manage scheduling for initial assessments and consultations.
  • Maintain up-to-date knowledge of hospice services, policies, and regulatory requirements.
  • Assist with administrative tasks to support the overall efficiency of the intake department.

Required Skills

  • Proven administrative or customer service experience, preferably within a healthcare setting.
  • Exceptional communication and interpersonal skills, with a compassionate and patient-centred approach.
  • Strong organizational skills and attention to detail.
  • Proficiency in using electronic health records (EHR) systems and Microsoft Office Suite.
  • Ability to handle sensitive information with the utmost discretion and confidentiality.
  • Demonstrated empathy and emotional resilience.

Preferred Qualifications

  • Previous experience in hospice, palliative care, or a similar medical intake role.
  • Understanding of medical terminology related to end-of-life care.
  • NVQ Level 3 in Business Administration or equivalent.
  • Experience working with vulnerable adults or individuals in crisis.
