Job Description

We are seeking an exceptionally skilled and dedicated Transplant Coordinator (RN) to join our leading transplant team. This is a high-responsibility role crucial to the success of our transplant programs, demanding expertise, empathy, and meticulous coordination. The successful candidate will manage the complex care continuum for transplant patients, from initial evaluation through post-transplant follow-up. This position offers a top salary commensurate with the critical nature of the role and the extensive expertise required. Key Responsibilities

  • Coordinate all aspects of the transplant process, including patient evaluation, listing, education, and follow-up care.
  • Serve as the primary point of contact for transplant candidates and their families, providing comprehensive education and emotional support.
  • Facilitate communication and collaboration among multidisciplinary team members, including physicians, surgeons, social workers, dietitians, and financial counselors.
  • Ensure timely and accurate completion of patient assessments, diagnostic tests, and necessary documentation for transplant listing.
  • Monitor patient status, laboratory results, and clinical progress, reporting critical findings to the transplant physician.
  • Educate patients on immunosuppressive regimens, potential complications, and lifestyle modifications post-transplant.
  • Participate in transplant selection committee meetings and patient care conferences.
  • Adhere to all regulatory guidelines and standards set by UNOS (United Network for Organ Sharing) and other governing bodies.
  • Provide on-call coverage for transplant patient inquiries and urgent clinical situations as required.

Required Skills

  • Current California Registered Nurse (RN) license
  • Bachelor of Science in Nursing (BSN)
  • Minimum of 3 years of clinical experience in transplant, critical care, nephrology, or cardiology
  • Demonstrated expertise in patient education and counseling
  • Excellent written and verbal communication skills
  • Strong organizational and critical thinking abilities
  • Proficiency in electronic health record (EHR) systems

Preferred Qualifications

  • Master of Science in Nursing (MSN)
  • Certified Clinical Transplant Coordinator (CCTC) certification
  • Experience with specific organ transplant programs (e.g., kidney, liver, heart)
  • Bilingual proficiency (especially Spanish)
