Job Description
We are seeking a dedicated and compassionate Hospice Nurse (RN) to join our growing team. This vital role involves providing direct patient care in the comfort of their own homes, ensuring a dignified and supportive experience for individuals facing life-limiting illnesses. As a Hospice Nurse, you will manage a caseload of patients, conducting regular home visits to assess needs, administer medications, provide symptom management, and offer emotional support to both patients and their families. Key Responsibilities
- Conduct comprehensive initial and ongoing nursing assessments of patients in their homes.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and families.
- Administer medications, provide wound care, and manage symptoms to ensure patient comfort and quality of life.
- Educate patients and caregivers on disease progression, symptom management, and end-of-life care options.
- Provide emotional and psychological support to patients and families, addressing grief and bereavement needs.
- Accurately document all patient encounters, observations, and interventions in the electronic medical record.
- Coordinate care with physicians, social workers, chaplains, and other team members.
- Participate in on-call rotation as required to ensure 24/7 patient support.
- Adhere to all hospice regulations, professional standards, and company policies.
Required Skills
- Current and unrestricted Registered Nurse (RN) license in the state of Kansas.
- Minimum of 2 years of experience as a Registered Nurse.
- Current Basic Life Support (BLS) certification.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
- Ability to work independently and manage a diverse caseload.
- Strong critical thinking and problem-solving abilities.
- Proficiency in using electronic medical records (EMR) systems.
- Commitment to providing holistic and patient-centered care.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) degree.
- Certified Hospice and Palliative Nurse (CHPN) certification.
- Previous experience in hospice, home health, or palliative care settings.
- Experience with wound care and pain management techniques.
