Job Description

We are seeking highly skilled and compassionate Advanced Practice Professionals to join our dynamic teams across various specialties within our Antioch medical centers. These roles are critical in delivering comprehensive, integrated patient care, spanning from primary care to highly specialized services. Successful candidates will leverage their advanced clinical knowledge and skills to assess, diagnose, treat, and manage patient conditions, ensuring the highest standards of care and patient satisfaction. Key Responsibilities

  • Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic testing.
  • Formulate diagnoses and develop individualized treatment plans in collaboration with supervising physicians and interdisciplinary teams.
  • Prescribe medications, order and interpret diagnostic tests, and perform therapeutic procedures within scope of practice.
  • Educate patients and their families on disease processes, treatment options, and preventive care strategies.
  • Provide ongoing management of chronic and acute conditions, ensuring continuity of care.
  • Participate in quality improvement initiatives, contributing to best practices and patient safety.
  • Maintain accurate and timely electronic health records, adhering to all regulatory and organizational standards.
  • Collaborate effectively with physicians, nurses, and other healthcare professionals to optimize patient outcomes.

Required Skills

  • Current California Advanced Practice RN (APRN) or Physician Assistant (PA) license.
  • Board certification in relevant specialty (e.g., Family Medicine, Internal Medicine, Pediatrics, Acute Care).
  • Master's or Doctoral degree from an accredited program for APRN/PA.
  • Strong clinical assessment, diagnostic, and therapeutic skills.
  • Excellent communication, interpersonal, and team collaboration abilities.
  • Proficiency in electronic health record (EHR) systems.
  • Demonstrated ability to work autonomously and manage a diverse patient caseload.

Preferred Qualifications

  • Minimum of 2 years of experience in an advanced practice role within a hospital or large clinic setting.
  • Experience with Kaiser Permanente systems and protocols.
  • Bilingual proficiency (especially Spanish) is a plus.
  • Participation in professional organizations and continuing education relevant to specialty.
