Job Description

We are seeking a dedicated and compassionate Gerontology Nurse Practitioner to join our dynamic Senior Care team in Burbank, CA. As a Gerontology Nurse Practitioner – Senior Care Specialist, you will play a crucial role in providing comprehensive primary and specialized care to our elderly members, focusing on health promotion, disease prevention, diagnosis, and management of acute and chronic conditions common in older adults. Key Responsibilities

  • Conduct comprehensive geriatric assessments, including physical exams, psychosocial evaluations, and cognitive screenings.
  • Diagnose and treat acute and chronic health conditions in older adult patients.
  • Develop and implement individualized care plans, focusing on age-appropriate interventions and health goals.
  • Prescribe medications, treatments, and therapies in accordance with state laws and clinical guidelines.
  • Order and interpret diagnostic tests, integrating findings into patient care management.
  • Provide patient and family education on disease management, healthy aging, medication adherence, and preventive care.
  • Collaborate with a multidisciplinary healthcare team, including physicians, registered nurses, social workers, and other specialists, to ensure coordinated and holistic patient care.
  • Participate in case conferences and team meetings to review patient progress and adjust care plans as needed.
  • Maintain accurate and thorough electronic health records (EHR) documentation.
  • Advocate for the needs of senior patients, ensuring access to appropriate resources and support services.
  • Stay current with best practices and advancements in geriatric medicine and nursing through continuing education.

Required Skills

  • Current California Registered Nurse (RN) license.
  • Current California Nurse Practitioner (NP) license.
  • National Board Certification as a Gerontological Nurse Practitioner (GNP) or Adult-Gerontology Nurse Practitioner (AGNP).
  • Master's or Doctor of Nursing Practice (DNP) degree from an accredited program.
  • DEA registration and prescriptive authority.
  • Minimum of 3 years of clinical experience as a Nurse Practitioner, preferably in geriatrics, internal medicine, or family medicine with a significant older adult patient panel.
  • Proficiency in electronic health record (EHR) systems.
  • Excellent clinical assessment, diagnostic, and treatment planning skills.
  • Strong communication, interpersonal, and team collaboration abilities.
  • Demonstrated empathy, patience, and cultural competence in working with older adults and their families.
  • Ability to work independently and manage a diverse patient caseload effectively.

Preferred Qualifications

  • Experience within an integrated healthcare delivery system.
  • Bilingual skills (e.g., Spanish, Armenian, Korean) relevant to the local community.
  • Advanced certifications in specific geriatric sub-specialties.
  • Leadership experience in healthcare settings or quality improvement initiatives.
