Job Description

EveryStep is seeking a compassionate and skilled Hospice Nurse (RN) to join our dedicated team in Des Moines, IA. This critical role involves providing personalized, holistic care to patients and their families in the comfort of their own homes. As a Hospice Nurse with EveryStep, you will manage a caseload of patients, assessing their needs, developing and implementing care plans, and providing symptom management and emotional support. You will educate families, coordinate with interdisciplinary team members, and ensure the highest quality of care during a sensitive time. We understand the value of your commitment, which is why this position includes mileage reimbursement for all home visits.

Key Responsibilities

  • Conduct comprehensive nursing assessments and re-assessments of hospice patients.
  • Develop, implement, and evaluate individualized care plans in collaboration with the patient, family, and interdisciplinary team.
  • Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
  • Provide expert symptom management and pain control.
  • Educate patients and families on disease progression, comfort care measures, and end-of-life planning.
  • Offer emotional and spiritual support, referring to social workers or chaplains as needed.
  • Document all patient care accurately and timely in accordance with regulatory standards.
  • Participate in interdisciplinary team meetings and contribute to care plan revisions.
  • Provide on-call coverage as scheduled.
  • Ensure compliance with all federal, state, and local regulations and agency policies.

Required Skills

  • Current, unencumbered Registered Nurse (RN) license in the State of Iowa
  • Minimum of 2 years of clinical nursing experience, preferably in hospice, home health, or acute care
  • Demonstrated ability to perform comprehensive nursing assessments and develop care plans
  • Excellent communication, interpersonal, and organizational skills
  • Strong problem-solving abilities and critical thinking skills
  • Ability to work autonomously and manage a demanding caseload
  • Valid driver's license, reliable transportation, and current auto insurance
  • Compassionate and empathetic approach to patient and family care

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Certified Hospice and Palliative Nurse (CHPN) certification
  • Previous experience with electronic health records (EHR) systems specific to home health or hospice
