Job Description
As a Nurse Health Coach focusing on Remote Wellness Strategy, you will be a vital part of our commitment to preventative care and member well-being. This 100% remote role involves empowering members to achieve their health goals through personalized coaching, education, and support. You will leverage your clinical nursing expertise to develop and implement individualized wellness plans, focusing on chronic disease management, lifestyle modifications, and health promotion. This flexible position allows you to make a significant impact on members’ lives from the comfort of your home office, utilizing virtual platforms and telecommunication tools to deliver exceptional care.
Key Responsibilities
- Conduct virtual health assessments and develop personalized health coaching plans.
- Provide evidence-based guidance on nutrition, exercise, stress management, and chronic condition self-management.
- Monitor member progress, adjust plans as needed, and provide ongoing encouragement and support.
- Collaborate with primary care providers and other healthcare professionals to ensure coordinated care.
- Educate members on health risks, preventative screenings, and community resources.
- Maintain accurate and confidential electronic health records for all member interactions.
- Act as a resource for members regarding wellness tools and programs offered by Kaiser Permanente.
- Participate in continuous professional development and training relevant to health coaching and remote care delivery.
Required Skills
- Active Registered Nurse (RN) license in at least one U.S. state (multi-state compact preferred).
- Minimum of 2 years of clinical nursing experience.
- Demonstrated experience in patient education, counseling, or health coaching.
- Excellent verbal and written communication skills, with strong active listening abilities.
- Proficiency in using virtual communication platforms (video conferencing, secure messaging) and electronic health records (EHR).
- Ability to work independently and manage a caseload effectively in a remote environment.
- Strong empathy, motivational interviewing skills, and a patient-centered approach.
- Reliable high-speed internet connection and a dedicated home office space.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Certification in Health Coaching (e.g., NBC-HWC, CHC).
- Experience with telehealth or remote patient monitoring.
- Knowledge of chronic disease management protocols and preventative care guidelines.
- Bilingual proficiency (especially Spanish).
