Job Description
We are seeking a dedicated and compassionate Hospice Nurse (RN) to provide high-quality, patient-centered care in the comfort of patients’ homes within the Henderson, Nevada area. This role involves managing a patient caseload, assessing their needs, implementing care plans, and providing emotional support to patients and their families. The ideal candidate will be an experienced Registered Nurse with a passion for hospice care and a commitment to maintaining dignity and comfort for our patients. Mileage reimbursement is provided for all travel related to home visits.
Key Responsibilities
- Conduct initial and ongoing assessments of patient needs, including physical, emotional, and spiritual well-being.
- Develop, implement, and evaluate individualized care plans in collaboration with the patient, family, and interdisciplinary team.
- Administer medications and treatments as prescribed, monitoring for effectiveness and adverse reactions.
- Provide skilled nursing care, including wound care, symptom management, and pain control.
- Educate patients and families on disease progression, comfort measures, and end-of-life care options.
- Document all patient visits and interactions accurately and thoroughly in the electronic medical record.
- Communicate effectively with physicians, caregivers, and other members of the hospice team.
- Provide emotional support and counseling to patients and families, addressing grief and bereavement issues.
- Participate in on-call rotation as required.
Required Skills
- Current unencumbered Registered Nurse (RN) license in the State of Nevada.
- Minimum of 2 years of clinical nursing experience, preferably in hospice, home health, or acute care.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage a caseload effectively.
- Proficiency with electronic medical records (EMR) systems.
- Valid driver's license and reliable transportation.
- Demonstrated empathy, compassion, and sensitivity to the needs of terminally ill patients and their families.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Certified Hospice and Palliative Nurse (CHPN) certification.
- Experience with hospice regulations and compliance.
- Familiarity with local community resources for hospice patients.
