Job Description
We are seeking a compassionate and skilled Hospice & Palliative Care Nurse to join our dedicated team in Charlotte, NC. This role is crucial in providing specialized nursing care, comfort, and support to patients facing life-limiting illnesses and their families. The ideal candidate will possess a deep understanding of palliative care principles, excellent communication skills, and a commitment to patient-centered care. Key Responsibilities
- Provide comprehensive nursing assessments, planning, and implementation of care for hospice and palliative care patients in various settings (home, facility, hospital).
- Administer medications, manage symptoms, and perform nursing procedures according to established protocols and physician orders.
- Educate patients and families on disease progression, symptom management, and end-of-life care options.
- Collaborate with an interdisciplinary team, including physicians, social workers, spiritual counselors, and therapists, to develop and revise individualized care plans.
- Offer emotional support and grief counseling to patients and their families.
- Maintain accurate and timely patient records, ensuring compliance with all regulatory standards.
- Participate in on-call rotation as required to provide 24/7 patient support.
- Advocate for patient comfort, dignity, and quality of life.
Required Skills
- Current unencumbered Registered Nurse (RN) license in North Carolina (or eligible for licensure via endorsement or compact state).
- Minimum of 1 year of clinical experience as an RN.
- Basic Life Support (BLS) certification.
- Demonstrated knowledge of hospice and palliative care principles.
- Excellent assessment, critical thinking, and problem-solving skills.
- Strong interpersonal and communication skills, with the ability to empathize and build rapport with patients and families.
- Ability to work independently and as part of a collaborative team.
- Valid driver's license and reliable transportation.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Experience in hospice, palliative care, oncology, or home health settings.
- Certified Hospice and Palliative Nurse (CHPN) certification.
- Familiarity with electronic health record (EHR) systems specific to hospice care.
