Job Description
We are seeking a dedicated and detail-oriented Medication History Technician to join our hospital-based team in Pomona, CA. This crucial role involves direct patient interaction to obtain comprehensive and accurate medication histories, ensuring patient safety and supporting clinical decision-making upon admission. You will work closely with pharmacists, nurses, and physicians in a fast-paced clinical environment, gaining invaluable exposure to hospital operations and patient care processes.
Key Responsibilities
- Conduct thorough patient interviews to accurately obtain and document current medication histories, including prescription, over-the-counter, herbal, and supplement use.
- Verify medication histories with pharmacies, family members, and other healthcare providers as needed.
- Enter accurate medication information into the electronic health record (EHR) system.
- Identify and report discrepancies or potential medication-related issues to pharmacists or nurses.
- Collaborate effectively with interdisciplinary healthcare teams to ensure seamless patient care transitions.
- Educate patients and families on medication reconciliation processes as appropriate.
- Maintain confidentiality of patient information and adhere to all hospital policies and regulatory requirements.
Required Skills
- Current California Pharmacy Technician License (CPhT)
- Strong interviewing and communication skills with diverse patient populations
- Exceptional attention to detail and accuracy
- Proficiency in medical terminology and common medication classes
- Ability to work independently and as part of a team in a fast-paced environment
- Basic computer literacy and experience with electronic health records (EHR) systems
Preferred Qualifications
- Previous experience as a Pharmacy Technician in a hospital or clinical setting (1-2 years)
- Experience with Epic EHR system
- Bilingual (English/Spanish) skills
