Job Description

We are seeking an experienced and highly motivated Registered Nurse (RN) to join our esteemed transplant team as a Transplant Coordinator. This critical role involves coordinating all aspects of the transplant process, from evaluation to post-transplant care, for patients undergoing organ transplantation. The successful candidate will serve as a central point of contact for patients, families, and multidisciplinary healthcare teams, ensuring seamless communication, education, and support throughout the complex transplant journey. This position demands exceptional clinical judgment, strong organizational skills, and a profound commitment to patient advocacy, reflecting the high responsibility inherent in improving and saving lives. This is an on-site position at our state-of-the-art facility in the Upper East Side, New York.

Key Responsibilities

  • Manage and coordinate the care of pre-transplant candidates, including screening, evaluation, and waitlist management.
  • Educate patients and families regarding the transplant process, medications, lifestyle changes, and potential complications.
  • Coordinate organ offers and facilitate the transplant procedure, working closely with surgeons, physicians, and other team members.
  • Provide comprehensive post-transplant care coordination, monitoring patient outcomes, managing immunosuppression protocols, and addressing complications.
  • Participate in multidisciplinary team meetings, patient selection conferences, and quality improvement initiatives.
  • Maintain accurate and timely documentation in electronic health records, ensuring compliance with regulatory standards.
  • Act as a liaison between patients, families, referring physicians, and transplant team members.
  • Provide on-call coverage as required to support transplant urgent needs.

Required Skills

  • Current Registered Nurse (RN) license in New York State.
  • Bachelor of Science in Nursing (BSN) required; Master's preferred.
  • Minimum of 4 years of clinical nursing experience, with at least 2 years in critical care, organ specific specialty, or transplant nursing.
  • Strong understanding of organ transplantation, immunosuppression, and post-transplant complications.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work autonomously and collaboratively in a fast-paced, high-pressure environment.
  • Proficiency in electronic health record systems.
  • Demonstrated ability to manage complex patient caseloads effectively.

Preferred Qualifications

  • Certified Clinical Transplant Coordinator (CCTC) certification.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Experience with specific organ transplant programs (e.g., heart, lung, kidney, liver).
  • Bilingual proficiency (e.g., Spanish) is a plus.
