Job Description
Join UPMC Home Healthcare as a Licensed Practical Nurse (LPN) and make a profound difference in the lives of our patients by providing dedicated one-on-one care in the comfort of their own homes. As an LPN, you will be a vital part of our care team, delivering direct patient care, administering medications, monitoring patient conditions, and educating patients and their families. Key Responsibilities
- Provide direct nursing care to patients in their homes under the supervision of a Registered Nurse (RN).
- Administer medications and treatments as prescribed by physicians.
- Monitor and record vital signs, symptoms, and other patient health indicators.
- Assist patients with activities of daily living (ADLs) including personal hygiene, mobility, and feeding.
- Educate patients and their families on health maintenance, disease prevention, and medication adherence.
- Communicate effectively with the interdisciplinary healthcare team regarding patient status and any changes.
- Maintain accurate and timely patient records in compliance with UPMC policies and regulatory requirements.
- Adhere to all safety protocols and infection control procedures.
- Respond to patient emergencies and provide appropriate first aid or interventions as needed.
- Assist with wound care, catheter care, and other specialized nursing procedures as delegated and within scope of practice.
Required Skills
- Current active LPN license in the state of Pennsylvania.
- Valid driver's license and reliable transportation.
- Current Basic Life Support (BLS) certification.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage a patient caseload effectively.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- One year of experience in home healthcare, acute care, or long-term care setting.
- Experience with a variety of patient populations (e.g., geriatric, pediatric, post-surgical).
- Certification in IV therapy.
