Job Description

We are seeking a dedicated and compassionate Community Health Aide with a strong outreach focus to join our dynamic team. This full-time role operates Monday to Friday, providing essential support to patients within their own homes and community settings. You will be a vital link between our clinical teams and the community, helping to promote health, prevent illness, and provide practical support. This position is perfect for someone passionate about community health, eager to engage directly with individuals, and committed to improving health outcomes through direct patient contact and educational outreach.

Key Responsibilities

  • Conduct regular home visits to deliver personal care, monitor vital signs, and provide medication prompts as per care plans.
  • Support patients in maintaining independence, promoting their physical and mental well-being within their community.
  • Assist with health promotion activities, educating individuals and groups on healthy lifestyles, disease prevention, and access to local health services.
  • Act as a first point of contact for patients, reporting any changes in condition or concerns to the supervising nurse or healthcare professional.
  • Maintain accurate and confidential patient records, ensuring all interactions and observations are documented promptly and clearly.
  • Facilitate patient access to community resources and support networks, including social services, voluntary organisations, and local health initiatives.
  • Collaborate effectively with a multidisciplinary team, including nurses, therapists, and social workers, to ensure holistic patient care.
  • Participate in team meetings and training sessions to enhance skills and knowledge.
  • Manage caseloads efficiently, prioritising visits and tasks to meet patient needs and service demands.

Required Skills

  • Excellent communication and interpersonal skills, with an empathetic and compassionate approach.
  • Ability to work independently and as part of a team, demonstrating initiative and problem-solving skills.
  • Good organisational skills and ability to manage a varied workload.
  • Basic IT skills for record-keeping and communication.
  • Full UK driving license and access to a reliable vehicle for community travel.
  • Understanding of confidentiality and safeguarding principles.
  • Demonstrated commitment to patient-centred care.

Preferred Qualifications

  • NVQ Level 2 or 3 in Health and Social Care or equivalent qualification.
  • Previous experience in a community health or social care setting.
  • Knowledge of local community resources in Leicester and surrounding areas.
  • Experience with electronic patient record systems.
