Job Description

Key Responsibilities
- Provide direct personal care, including bathing, dressing, grooming, and mobility assistance, tailored to individual client needs.
- Assist clients with light housekeeping tasks, meal preparation, and medication reminders.
- Monitor and report changes in client's health status, behavior, or living conditions to the nursing supervisor.
- Engage clients in social activities and provide companionship to enhance their quality of life.
- Maintain accurate and timely documentation of care provided and client observations.
- Adhere to all health and safety guidelines, as well as Baystate Health policies and procedures.
- Foster a safe, supportive, and respectful environment for clients in their homes.
Required Skills
- Current Home Health Aide (HHA) certification in Massachusetts.
- Demonstrated compassion, empathy, and patience.
- Excellent verbal and written communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and a valid driver's license.
- Physical ability to perform duties, including lifting and assisting with transfers.
Preferred Qualifications
- Previous experience working with elderly or disabled individuals.
- Experience in a home health or hospice setting.
- Knowledge of basic first aid and emergency procedures.
