Job Description
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a rewarding Full-Time position in North Raleigh, NC. This role focuses on providing individualized, one-on-one patient care in a home setting, allowing you to build meaningful relationships with your patients and make a significant impact on their recovery and daily lives. You will be an integral part of our team, delivering skilled nursing care, monitoring patient conditions, and ensuring comfort and safety. Key Responsibilities
- Provide direct skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN).
- Administer medications and treatments as prescribed by a physician.
- Monitor and record vital signs, observe patient conditions, and report changes to the supervising RN and physician.
- Assist patients with activities of daily living (ADLs) as needed.
- Educate patients and their families on health maintenance, medication administration, and disease management.
- Maintain accurate and timely clinical documentation using electronic health records (EHR).
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
- Ensure patient safety and comfort within the home environment.
- Perform wound care, catheter care, ostomy care, and other skilled procedures as per scope of practice.
Required Skills
- Current, unencumbered LPN license in North Carolina.
- Valid Basic Life Support (BLS) certification.
- Demonstrated ability to provide compassionate, patient-centered care.
- Excellent communication and interpersonal skills.
- Strong organizational skills and attention to detail.
- Proficiency in electronic health record (EHR) documentation.
- Ability to work independently and manage a patient caseload effectively.
- Reliable transportation and valid driver's license with proof of auto insurance.
Preferred Qualifications
- One year of experience in home health, acute care, or long-term care setting.
- Experience with various patient populations and complex medical conditions.
- Familiarity with HCHB (Home Care Home Base) or similar home health software.
- Certification in specific areas such as IV therapy or wound care.
