Job Description

Join the compassionate and dedicated team at The University of Vermont Medical Center as a Medication History Technician. In this vital hospital-based role, you will be instrumental in ensuring patient safety and continuity of care by accurately collecting and documenting patient medication histories. You will work closely with pharmacists, physicians, nurses, and patients, providing critical support in a fast-paced clinical environment. This position offers significant clinical exposure and the opportunity to contribute directly to positive patient outcomes.

Key Responsibilities

  • Obtain accurate and complete medication histories from patients, family members, caregivers, and external healthcare providers.
  • Document medication histories in the electronic health record (EHR) in a timely and efficient manner.
  • Clarify discrepancies and missing information with patients, prescribers, and pharmacies.
  • Collaborate with pharmacists, nurses, and physicians to reconcile medication lists upon admission, transfer, and discharge.
  • Educate patients and families on medication safety and the importance of accurate medication lists.
  • Maintain strict confidentiality of patient information.
  • Adhere to all hospital policies, procedures, and regulatory requirements.
  • Participate in quality improvement initiatives related to medication history processes.

  • Basic understanding of medical terminology and common medications.

Preferred Qualifications

  • Certified Pharmacy Technician (CPhT) certification.
  • Previous experience as a Pharmacy Technician or Medical Assistant, especially in a hospital or clinical setting.
  • Associate's degree in a healthcare-related field.
  • Experience with Epic EHR system.
