Job Description
Join Optum as a Remote Nurse Health Coach and empower individuals to achieve their wellness goals from the comfort of your home. This flexible, full-time role allows you to leverage your nursing expertise to guide members through personalized health plans, focusing on preventive care, chronic disease management, and lifestyle modifications. You will be a vital resource, providing education, motivation, and support to foster lasting healthy habits and improve overall quality of life. We are seeking a compassionate and proactive Registered Nurse who thrives in a remote environment and is passionate about driving positive health outcomes through strategic coaching.
Key Responsibilities
- Conduct comprehensive health assessments and develop individualized care plans for members.
- Provide telephonic and virtual health coaching to members on various health topics including nutrition, exercise, stress management, and chronic condition management.
- Educate members on disease processes, medication adherence, and self-care strategies.
- Motivate and support members in setting and achieving realistic health goals.
- Monitor member progress, adjust care plans as needed, and document all interactions accurately and timely.
- Collaborate with a multidisciplinary team to ensure coordinated and holistic care.
- Adhere to all HIPAA regulations and maintain strict confidentiality of member information.
- Participate in ongoing training and professional development to stay current with best practices in health coaching and nursing.
Required Skills
- Current, unrestricted Registered Nurse (RN) license in the state of Washington.
- Minimum 2 years of clinical nursing experience.
- Strong understanding of chronic disease management principles and preventive care.
- Excellent verbal and written communication skills with the ability to build rapport quickly.
- Proficiency in using electronic health records (EHR) systems and virtual communication platforms.
- Ability to work independently, manage a caseload, and prioritize tasks effectively in a remote setting.
- Empathy, patience, and strong motivational interviewing skills.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Certification in Health Coaching (e.g., NB-HWC, CHC).
- Experience in remote health coaching, case management, or disease management programs.
- Familiarity with various wellness methodologies and behavior change theories.
