Job Description
We are seeking a dedicated and compassionate Community Psychiatric Nurse (CPN) to join our dynamic community mental health team in Manchester. This is an exciting opportunity for an experienced mental health nurse to provide high-quality, evidence-based care to individuals with complex mental health needs in a community setting. You will be instrumental in supporting patients’ recovery journeys, working collaboratively with multidisciplinary teams and external agencies.
Key Responsibilities
- Conduct comprehensive mental health assessments, risk assessments, and develop individualized care plans.
- Provide skilled nursing interventions, including medication management, psychological support, and psychoeducation.
- Manage a caseload of patients with diverse and complex mental health needs, ensuring continuity of care.
- Collaborate effectively with service users, their families, carers, and other healthcare professionals within a multidisciplinary team.
- Participate in crisis intervention and provide support to service users experiencing acute mental health episodes.
- Maintain accurate and up-to-date patient records in accordance with Trust policies and NMC guidelines.
- Contribute to the ongoing development and evaluation of community mental health services.
- Act as a mentor and provide supervision to junior staff and students where appropriate.
Required Skills
- Registered Mental Health Nurse (RMN) with current NMC registration.
- Degree in Mental Health Nursing or equivalent qualification.
- Demonstrable experience working in a community mental health setting.
- Excellent assessment, care planning, and risk management skills.
- Strong communication, interpersonal, and teamwork abilities.
- Knowledge of relevant mental health legislation and best practice guidelines.
- Ability to work autonomously and as part of a team.
Preferred Qualifications
- Post-registration experience in a specific mental health specialty (e.g., CBT, severe mental illness).
- Evidence of continuing professional development relevant to community mental health nursing.
- Experience of supervising junior staff or students.
- Valid UK driving license and access to a car for work purposes (mileage reimbursement provided).
