Job Description

We are seeking a dedicated and detail-oriented Medication Reconciliation Technician with a strong focus on clinical data to join our pharmacy team. In this critical role, you will be responsible for obtaining and verifying accurate medication histories for patients upon admission, transfer, and discharge. Your primary focus will be on ensuring the integrity and completeness of clinical medication data within our electronic health record (EHR) systems, identifying discrepancies, and collaborating with healthcare providers to resolve them. This position plays a vital role in patient safety by preventing medication errors and supporting optimal patient outcomes through meticulous data management.

Key Responsibilities

  • Obtain comprehensive and accurate medication histories from patients, families, and external healthcare providers.
  • Verify medication information, including drug name, dosage, frequency, route, and last dose taken, through various sources.
  • Identify and document discrepancies, potential drug interactions, and allergies, escalating critical issues to pharmacists or prescribing clinicians.
  • Input and maintain precise medication data within the electronic health record (EHR) system.
  • Collaborate with pharmacists, nurses, and physicians to resolve medication history discrepancies and clarify orders.
  • Educate patients and families on the importance of medication adherence and reconciliation processes.
  • Participate in quality improvement initiatives related to medication safety and clinical data accuracy.
  • Maintain strict confidentiality of patient information.
  • Assist with other pharmacy support duties as needed.

Required Skills

  • Certified Pharmacy Technician (CPhT) certification.
  • Minimum of 1 year of experience in a hospital or clinical pharmacy setting.
  • Proficiency with electronic health record (EHR) systems and pharmacy software.
  • Excellent verbal and written communication skills.
  • Strong attention to detail and accuracy in data entry.
  • Ability to work independently and as part of a multidisciplinary team.
  • Knowledge of medical terminology, drug names, and common dosages.

Preferred Qualifications

  • Associate's degree in a healthcare-related field.
  • Experience specifically in medication reconciliation roles.
  • Familiarity with HCA Florida Healthcare's specific EHR system (e.g., Meditech, Epic).
  • Basic understanding of clinical data analysis and reporting.
