Job Description
We are seeking a dedicated and detail-oriented Medication History Technician to join our hospital-based team in Huntington Beach, CA. This crucial role involves obtaining and documenting accurate medication histories for patients upon admission, collaborating closely with nurses, pharmacists, and physicians. You will be instrumental in preventing medication errors and ensuring patient safety through thorough and precise information gathering. This position offers significant clinical exposure within a dynamic hospital environment, ideal for individuals looking to grow their healthcare career.
Key Responsibilities
- Obtain comprehensive and accurate medication histories from patients, their families, and external healthcare providers upon admission.
- Document medication histories meticulously into the electronic health record (EHR) in a timely manner.
- Clarify discrepancies or missing information with patients, family members, pharmacies, and prescribers.
- Communicate effectively with pharmacists, nurses, and physicians regarding medication history findings and potential discrepancies.
- Participate in medication reconciliation processes to ensure continuity of care.
- Educate patients and families on medication-related issues as appropriate and directed by clinical staff.
- Adhere to all hospital policies, procedures, and regulatory requirements related to medication management and patient safety.
- Contribute to quality improvement initiatives related to medication history taking.
Required Skills
- High School Diploma or GED
- Certification as a Pharmacy Technician (CPhT) or equivalent
- Minimum 1 year of experience as a Pharmacy Technician, preferably in a hospital or retail setting
- Strong understanding of pharmaceutical terminology, drug names (generic and brand), and dosage forms
- Excellent communication and interpersonal skills
- Proficiency in electronic health record (EHR) systems
- Exceptional attention to detail and accuracy
- Ability to work independently and as part of a multidisciplinary team
Preferred Qualifications
- Associate's degree in a related healthcare field
- Experience specifically in medication history taking or medication reconciliation
- Familiarity with Cerner or Epic EHR systems
- Bilingual proficiency (English/Spanish)
