Job Description
Join Sentara Healthcare as an Entry Level Patient Navigator with a focus on Healthcare Support. In this crucial role, you will be the primary point of contact for patients, guiding them through the complexities of the healthcare system, assisting with appointments, understanding treatment plans, and connecting them with necessary resources. This position is ideal for individuals passionate about helping others, eager to learn, and looking to start a rewarding career in healthcare. You will work closely with clinical teams to ensure patients receive timely, coordinated, and compassionate care.
Key Responsibilities
- Assist patients with scheduling appointments, navigating care pathways, and understanding medical information.
- Act as a liaison between patients and healthcare providers, ensuring effective communication.
- Connect patients with community resources, support groups, and financial assistance programs as needed.
- Document patient interactions and progress accurately in electronic health records.
- Educate patients on their treatment plans, medication adherence, and preventive care.
- Identify potential barriers to care and collaborate with the care team to find solutions.
- Maintain strict patient confidentiality and adhere to all HIPAA regulations.
- Contribute to a positive and supportive patient experience.
Required Skills
- High School Diploma or GED
- Excellent verbal and written communication skills
- Strong interpersonal skills with a compassionate and empathetic approach
- Basic computer proficiency and ability to learn new software systems
- Exceptional organizational skills and attention to detail
- Ability to work effectively in a fast-paced healthcare environment
- Demonstrated ability to maintain confidentiality
Preferred Qualifications
- Associate's or Bachelor's degree in a healthcare-related field (e.g., Public Health, Social Work, Healthcare Administration)
- Previous experience in a customer service or patient-facing role
- Familiarity with medical terminology
- Bilingual proficiency (especially Spanish)
