Job Description
Join Fairview Health Services as a Home Health Aide in our Team Based Support Environment. In this vital role, you will provide compassionate, high-quality care to patients in their homes, assisting them with daily living activities, ensuring their comfort, and promoting their independence. You will work closely with a multidisciplinary team to implement care plans and monitor patient progress, ensuring holistic support.
Key Responsibilities
- Assist patients with personal care activities, including bathing, dressing, grooming, and toileting.
- Support patients with mobility and transfers, using proper body mechanics and assistive devices as needed.
- Prepare nutritious meals and assist with feeding, ensuring dietary restrictions are followed.
- Perform light housekeeping tasks, such as laundry, tidying common areas, and ensuring a safe home environment.
- Monitor and report changes in patient's physical, mental, or emotional condition to the supervising nurse or care coordinator.
- Provide companionship and emotional support to patients, fostering a trusting and respectful relationship.
- Assist with medication reminders as per care plan and training.
- Accurately document services provided and observations according to agency policies and procedures.
- Communicate effectively with patients, families, and the healthcare team to ensure coordinated care.
- Adhere to all safety protocols, infection control procedures, and patient privacy regulations.
Required Skills
- High School Diploma or GED
- Valid Home Health Aide (HHA) certification or completion of an approved HHA training program
- Basic Life Support (BLS) certification (or ability to obtain within 30 days of hire)
- Excellent communication and interpersonal skills
- Ability to follow written and verbal instructions accurately
- Compassionate and patient-centered approach to care
- Reliable transportation and valid driver's license
- Ability to perform physical tasks required for patient care (lifting, bending, standing)
Preferred Qualifications
- Previous experience in home healthcare or a similar caregiving role
- Familiarity with electronic health records (EHR) systems
- Multilingual abilities, particularly in languages common in the Saint Paul area
- CPR certification
