Job Description
Maxim Healthcare Services is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a rewarding home care position in Baltimore, MD. This role offers the unique opportunity to provide one-on-one patient care, focusing entirely on the needs of a single individual in their private residence. As an LPN, you will be instrumental in delivering personalized medical care, monitoring patient conditions, administering medications, and collaborating with a multidisciplinary team to ensure the highest quality of life for our patients. Key Responsibilities
- Provide direct patient care in accordance with the physician's plan of care, focusing on one-on-one attention.
- Administer prescribed medications and treatments, observing and documenting patient responses.
- Monitor vital signs, observe physical and mental conditions, and report changes to the supervising RN or physician.
- Perform skilled nursing procedures such as wound care, g-tube feedings, and catheter care.
- Educate patients and their families on health maintenance and disease prevention.
- Maintain accurate and timely clinical records and documentation.
- Assist with activities of daily living (ADLs) as needed.
- Ensure patient safety and comfort within the home environment.
- Communicate effectively with patients, families, and the healthcare team.
Required Skills
- Current and active LPN license in the state of Maryland
- Current BLS/CPR certification
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Compassionate and patient-focused approach to care
Preferred Qualifications
- Minimum of 1 year of LPN experience, preferably in home health, acute care, or skilled nursing
- Experience with electronic medical records (EMR) systems
- Knowledge of common chronic conditions (e.g., diabetes, respiratory issues)
- Punctual and reliable transportation
