Job Description
We are seeking a meticulous and dedicated Medication Reconciliation Technician with a strong focus on clinical data to join our growing team. In this critical role, you will be responsible for obtaining accurate medication histories from patients, families, and external healthcare providers. Your expertise will ensure the precise documentation and reconciliation of medications upon admission, transfer, and discharge, significantly contributing to patient safety and positive health outcomes. This position requires keen attention to detail, excellent communication skills, and a commitment to maintaining high standards of data integrity within our electronic health record systems.
Key Responsibilities
- Collect comprehensive medication histories from patients, caregivers, and pharmacies, ensuring accuracy and completeness.
- Verify and reconcile medication lists across various healthcare settings (admission, transfer, discharge).
- Identify and resolve discrepancies in medication information by collaborating with physicians, nurses, and pharmacists.
- Accurately document all medication reconciliation activities in the electronic health record (EHR) system.
- Educate patients and families on their current medication regimen as needed.
- Maintain up-to-date knowledge of pharmaceutical agents, common dosages, and drug interactions.
- Contribute to quality improvement initiatives related to medication safety and data management.
- Assist in the training and mentorship of new team members regarding medication reconciliation processes and EHR utilization.
Required Skills
- High School Diploma or GED
- Certification as a Pharmacy Technician (CPhT) or equivalent clinical experience
- Minimum 2 years of experience in a healthcare setting, preferably with medication reconciliation or pharmacy operations
- Proficiency in medical terminology and pharmaceutical nomenclature
- Exceptional attention to detail and accuracy
- Strong verbal and written communication skills
- Competency in using electronic health record (EHR) systems and other clinical software
- Ability to work independently and collaboratively in a fast-paced environment
Preferred Qualifications
- Associate's degree in a healthcare-related field
- Experience with Epic EHR system
- Previous experience specifically focused on clinical data analysis or quality improvement initiatives
- Bilingual (English/Spanish) skills
