Job Description
Optum is seeking a dedicated and compassionate Nurse Health Coach to join our remote team. This is a unique opportunity to empower individuals to achieve their health and wellness goals through personalized coaching, education, and support. As a crucial member of our wellness strategy team, you will leverage your nursing expertise to guide members through various health challenges, promote preventative care, and foster sustainable healthy behaviors. This role offers the flexibility of a remote work environment, allowing you to make a significant impact from anywhere.
Key Responsibilities
- Provide telephonic and virtual health coaching to members, focusing on chronic disease management, preventive care, and lifestyle modifications.
- Develop personalized health plans based on member assessments, health risks, and personal goals.
- Educate members on health conditions, medication adherence, nutrition, exercise, stress management, and other wellness topics.
- Motivate and support members in setting and achieving realistic health goals through empathetic and evidence-based coaching techniques.
- Collaborate with healthcare providers and care teams to ensure coordinated and comprehensive member care.
- Document all member interactions and progress accurately and timely in the electronic health record system.
- Participate in ongoing training and professional development to maintain current knowledge of health coaching best practices and clinical guidelines.
Required Skills
- Active, unrestricted Registered Nurse (RN) license in Florida (or compact state licensure with the ability to obtain FL license).
- Minimum 2 years of clinical nursing experience (e.g., medical-surgical, community health, home health).
- Excellent verbal and written communication skills.
- Strong interpersonal and motivational interviewing skills.
- Proficiency with electronic health records (EHR) and standard office software.
- Ability to work independently in a remote setting with strong organizational and time management skills.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Certification in Health Coaching (e.g., NB-HWC, CHES, or similar).
- Experience in telephonic health coaching or case management.
- Familiarity with chronic condition management programs (e.g., diabetes, hypertension, heart disease).
- Bilingual proficiency (especially Spanish).
