Job Description
We are seeking a dedicated and experienced Senior Support Worker to join our team, specializing in supporting individuals with learning disabilities in Prescott, AZ. This vital role involves providing direct person-centered care, promoting independence, and enhancing the quality of life for our service users. As a Senior Support Worker, you will be instrumental in developing and implementing personalized care plans, leading shifts, and mentoring junior staff. You will work closely with families and other healthcare professionals to ensure a holistic approach to care, championing the rights and dignity of every individual.
Key Responsibilities
- Provide direct personal care and support to individuals with learning disabilities, adhering to care plans and individual needs.
- Lead and supervise shifts, ensuring smooth operation and effective delegation of duties to junior support staff.
- Assist in the development, implementation, and review of individualized support plans, ensuring they are person-centered and outcomes-focused.
- Support individuals with daily living activities, community integration, social engagement, and skill development.
- Administer medication in accordance with policies and procedures, maintaining accurate records.
- Maintain clear and concise records, including daily notes, incident reports, and progress updates.
- Act as a key worker for designated individuals, building strong relationships and advocating for their needs.
- Collaborate with multi-disciplinary teams, families, and external agencies to ensure coordinated and comprehensive care.
- Identify and report any concerns regarding safeguarding or welfare of service users.
- Participate in ongoing training and professional development opportunities.
Required Skills
- Minimum of 4 years of experience working with individuals with learning disabilities.
- Demonstrated ability to lead and mentor a team.
- Excellent communication and interpersonal skills, with a compassionate and patient approach.
- Strong understanding of person-centered care principles.
- Ability to manage challenging behaviors effectively and calmly.
- Proficiency in record-keeping and report writing.
- Valid driver's license and access to reliable transportation.
Preferred Qualifications
- NVQ Level 3 in Health and Social Care or equivalent qualification.
- Experience in developing and reviewing care plans.
- Knowledge of relevant legislation and best practices in social care.
- First Aid and Basic Life Support certification.
