Job Description

We are seeking a dedicated and empathetic Hospice Intake Coordinator to join our team in Downtown Oceanside. This pivotal administrative role involves being the first point of contact for patients, families, and healthcare professionals seeking hospice services. You will be responsible for managing the intake process from initial inquiry to admission, ensuring a seamless and supportive transition for our patients. This role requires exceptional organizational skills, a compassionate demeanor, and the ability to handle sensitive information with the utmost discretion. Key Responsibilities

  • Manage all incoming calls and inquiries regarding hospice services with professionalism and empathy.
  • Conduct initial patient screenings, gathering essential medical and demographic information.
  • Verify patient eligibility for hospice care based on established criteria and insurance guidelines.
  • Coordinate with clinical teams, physicians, and other healthcare providers to facilitate patient admissions.
  • Process and maintain accurate patient records, ensuring compliance with all regulatory requirements.
  • Explain hospice services and benefits clearly to patients and families, addressing their questions and concerns.
  • Schedule and confirm admission appointments and initial assessments.
  • Collaborate with the interdisciplinary team to ensure a smooth transition of care.
  • Maintain confidentiality of all patient and company information.
  • Assist with other administrative tasks as needed to support the hospice team.

Required Skills

  • Minimum 1 year of administrative experience, preferably in a healthcare setting.
  • Exceptional interpersonal and communication skills, both verbal and written.
  • Strong organizational abilities and attention to detail.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Ability to work effectively in a fast-paced, emotionally sensitive environment.
  • Demonstrated empathy, compassion, and a non-judgmental approach.
  • Ability to multitask and prioritize effectively.

Preferred Qualifications

  • Associate's or Bachelor's degree in healthcare administration, business, or a related field.
  • Previous experience in hospice, home health, or palliative care.
  • Familiarity with medical terminology and electronic health record (EHR) systems.
  • Bilingual proficiency (English/Spanish) is a plus.
