Job Description

Join UnitedHealthcare as a Case Management RN and play a pivotal role in optimizing healthcare outcomes for our members, all from the comfort of your home. This is a 100% Work From Home (WFH) opportunity, offering flexibility while making a significant impact. As a Case Management RN, you will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to meet an individual’s health needs. You will advocate for our members, ensuring they receive appropriate, cost-effective care through the continuum of care. This role requires excellent clinical judgment, strong communication skills, and a passion for improving health outcomes. You will collaborate with patients, families, physicians, and other healthcare professionals to develop and implement care plans, manage resources, and facilitate smooth transitions of care.

Key Responsibilities

  • Conduct comprehensive assessments of member's health needs, including clinical, social, and emotional factors.
  • Develop, implement, and monitor individualized care plans in collaboration with members, families, and healthcare providers.
  • Coordinate healthcare services and resources to ensure continuity of care and appropriate utilization.
  • Educate members and families on health conditions, treatment options, and self-management strategies.
  • Evaluate the effectiveness of care plans and adjust as necessary to achieve desired outcomes.
  • Advocate for members to ensure access to necessary services and resources.
  • Document all interactions and interventions accurately and timely in the electronic health record.
  • Adhere to all company policies, procedures, and regulatory requirements.
  • Participate in interdisciplinary team meetings and collaborate with colleagues to optimize member care.

Required Skills

  • Current, unrestricted Registered Nurse (RN) license in the state of Florida (or ability to obtain).
  • Minimum of 3 years of clinical experience as a Registered Nurse.
  • Strong knowledge of case management principles and practices.
  • Excellent communication, interpersonal, and organizational skills.
  • Proficiency with computer systems, including electronic health records and Microsoft Office Suite.
  • Ability to work independently and manage a diverse caseload effectively in a remote environment.
  • Demonstrated ability to critically think and problem-solve complex clinical situations.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Certified Case Manager (CCM) certification.
  • Experience in managed care, home health, or community health.
  • Previous experience in a remote work environment.
  • Bilingual proficiency (especially Spanish).
