Job Description

We are seeking a compassionate and energetic Social Support Worker to join our team, specifically focusing on providing essential support to 18-year-old teens within the Jacksonville community. While the job title mentions ‘NDIS’, this role operates within the US disability services framework, applying principles of individualized support, community participation, and person-centered planning. The successful candidate will work directly with young adults, helping them develop life skills, access community resources, foster social connections, and achieve their personal goals. Key Responsibilities

  • Provide one-on-one and group support to 18-year-old teens with intellectual and developmental disabilities.
  • Assist individuals in developing and implementing personal goals related to independence, education, employment, and social skills.

  • Support individuals in navigating public transportation, managing personal finances, and developing daily living skills.
  • Advocate for clients' rights and needs, ensuring their voices are heard and respected.
  • Maintain accurate and timely documentation of services provided, client progress, and critical incidents.
  • Collaborate with families, caregivers, and other service providers to ensure holistic and coordinated support.
  • Promote a safe, inclusive, and empowering environment for all clients.
  • Attend regular team meetings, training sessions, and professional development opportunities.

Required Skills

  • Excellent verbal and written communication skills.

  • Ability to pass a comprehensive background check.

Preferred Qualifications

  • Associate's or Bachelor's degree in Social Work, Psychology, Education, or a related field.
  • Previous experience working with adolescents or young adults with intellectual and developmental disabilities.
  • Knowledge of local community resources and disability support services in Jacksonville, FL.
  • CPR and First Aid certification.
