Job Description
We are seeking a dedicated and empathetic Nurse Health Coach to join our remote wellness strategy team. In this flexible, full-time role, you will empower individuals to achieve their health and wellness goals through personalized coaching, education, and support. This position is 100% remote, allowing you to work from anywhere in the USA. You will leverage your nursing expertise to guide participants in developing sustainable lifestyle changes, managing chronic conditions, and navigating complex health information. This role emphasizes a proactive approach to health, focusing on preventive care and holistic well-being.
Key Responsibilities
- Provide one-on-one health coaching sessions via phone or video conference to a diverse patient population.
- Develop personalized health plans based on individual needs, health assessments, and goals.
- Educate participants on various health topics, including nutrition, exercise, stress management, and chronic disease prevention.
- Motivate and empower individuals to adopt healthy behaviors and maintain long-term lifestyle changes.
- Collaborate with healthcare providers and other team members to ensure coordinated care.
- Document participant interactions, progress, and outcomes accurately in designated systems.
- Stay updated on current health coaching methodologies, evidence-based practices, and wellness trends.
- Maintain strict confidentiality and adhere to all HIPAA regulations and company policies.
Required Skills
- Active Registered Nurse (RN) license in good standing (any US state, multi-state preferred).
- Minimum of 3 years of clinical nursing experience.
- Excellent verbal and written communication skills.
- Strong interpersonal skills with the ability to build rapport and trust quickly.
- Proficiency in motivational interviewing and active listening techniques.
- Demonstrated ability to work independently and manage a caseload effectively in a remote environment.
- Comfortable with technology and virtual communication platforms.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) preferred.
- Certification in Health Coaching (e.g., NB-HWC, CHC, etc.).
- Experience in telehealth, case management, or disease management.
- Bilingual skills (e.g., Spanish, Mandarin) are a plus.
- Experience with electronic health records (EHR) systems.
