Job Description

Key Responsibilities
- Provide direct support to service users, assisting them with daily living activities, medication management, and personal care as needed.
- Engage service users in therapeutic and recreational activities, fostering independence and promoting well-being.
- Support service users in developing life skills, accessing community resources, and achieving personal goals.
- Monitor and report on changes in service users' physical and mental health status to senior staff.
- Maintain accurate and timely documentation of observations, interactions, and care plans.
- Collaborate with a multidisciplinary team to ensure holistic and person-centered care.
- Adhere to all health and safety guidelines, policies, and procedures.
- Participate in ongoing training and professional development opportunities.
