Job Description

We are seeking a highly motivated and compassionate Public Health Nurse to join our team. In this full-time, on-site role, you will play a crucial part in delivering essential health services to the community. You will engage with diverse populations, conduct health assessments, provide education, and implement public health programs aimed at preventing disease and promoting wellness. This position offers a rewarding opportunity to serve the public and contribute to a healthier Marion County, working Monday through Friday.

Key Responsibilities

  • Conduct community health assessments and develop individualized care plans.
  • Provide health education on various topics, including chronic disease management, maternal and child health, and infectious disease prevention.
  • Administer immunizations and conduct screenings as part of public health initiatives.
  • Participate in disease surveillance, contact tracing, and outbreak investigations.
  • Collaborate with community partners, schools, and other agencies to implement health programs.
  • Maintain accurate and confidential client records in compliance with HIPAA regulations.
  • Provide direct nursing care and case management for individuals and families.
  • Advocate for public health policies and programs that benefit the community.
  • Respond to public health emergencies as part of a coordinated team.

Required Skills

  • Current Registered Nurse (RN) license in the State of Indiana.
  • Bachelor of Science in Nursing (BSN) from an accredited program.
  • Minimum of 2 years of experience in public health, community health, or a related field.
  • Strong understanding of public health principles and practices.
  • Excellent communication, interpersonal, and cultural competency skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in electronic health record (EHR) systems and standard office software.
  • Valid driver's license and reliable transportation.
  • Ability to conduct home visits and community outreach activities.

Preferred Qualifications

  • Master's degree in Public Health (MPH) or related field.
  • Certification in Public Health (CPH).
  • Experience working with diverse and underserved populations.
  • Fluency in a second language (e.g., Spanish, Burmese).
  • Experience with data collection and analysis for public health reporting.
