Job Description
We are seeking a compassionate and dedicated Hospice & Palliative Care Nurse to join our team in Sarnia. This crucial role involves providing holistic, patient-centered care to individuals and their families facing life-limiting illnesses. Key Responsibilities
- Assess, plan, implement, and evaluate nursing care for hospice and palliative care patients.
- Administer medications and treatments as prescribed, meticulously monitoring patient responses.
- Provide expert pain and symptom management in collaboration with the interdisciplinary team.
- Offer emotional, spiritual, and psychosocial support to patients and their families.
- Educate patients and families on disease progression, treatment options, and end-of-life care planning.
- Coordinate care with physicians, social workers, spiritual counselors, and other healthcare professionals.
- Maintain accurate and thorough patient records in compliance with professional standards and organizational policies.
- Advocate for patient comfort, dignity, and personal wishes.
- Participate in ongoing professional development and team meetings to enhance service delivery.
Required Skills
- Valid Registered Nurse (RN) license in Ontario or eligibility for licensure.
- Demonstrated experience in a clinical nursing setting (minimum 2 years).
- Exceptional communication and interpersonal skills, with a strong emphasis on empathy and active listening.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Proficiency in pain and symptom management techniques.
- Sound critical thinking and problem-solving abilities.
- Commitment to ethical practices and patient advocacy.
Preferred Qualifications
- Certification in Hospice Palliative Care (CHPCN(C)) or willingness to obtain.
- Experience specifically in hospice, palliative care, oncology, or geriatrics.
- Bachelor of Science in Nursing (BScN).
- Familiarity with electronic health record systems specific to Ontario healthcare.
- Understanding of grief and bereavement support principles.
