Job Description
We are seeking a compassionate and proactive Community Health Aide to join our outreach team in Kelowna. This vital role focuses on providing direct support and education to individuals and families within the community, often in their homes or other community settings. Working Monday to Friday, you will be instrumental in connecting clients with essential health services, promoting healthy living, and ensuring continuity of care. The ideal candidate will possess excellent communication skills, a strong empathetic approach, and a commitment to improving health outcomes for vulnerable populations.
Key Responsibilities
- Conduct regular outreach visits to clients in their homes and community settings to assess needs and provide support.
- Assist clients with basic personal care, medication reminders, and activities of daily living as needed.
- Provide health education and information on topics such as nutrition, hygiene, chronic disease management, and preventative care.
- Facilitate access to community resources, including social services, mental health support, housing assistance, and food security programs.
- Monitor and report changes in clients' health status or living conditions to the healthcare team.
- Maintain accurate and timely client records and documentation.
- Collaborate effectively with nurses, social workers, and other healthcare professionals to ensure coordinated client care.
- Advocate for clients' needs and rights within the healthcare system.
- Participate in team meetings, training, and professional development activities.
Required Skills
- High School Diploma or equivalent.
- Completion of a recognized Health Care Assistant or Community Support Worker program.
- Valid BC driver's license and access to a reliable vehicle.
- Excellent interpersonal and communication skills (verbal and written).
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational and time management abilities.
- Demonstrated empathy, patience, and cultural sensitivity.
- Basic computer proficiency for record-keeping and communication.
- Ability to lift, transfer, and reposition clients safely.
Preferred Qualifications
- Previous experience in community health, home care, or an outreach setting.
- Knowledge of local community resources in Kelowna and the Okanagan region.
- Experience working with diverse populations, including seniors, individuals with disabilities, and those facing socio-economic challenges.
- First Aid and CPR certification.
